Enrollment


Eligible colleagues must log on to www.ezenroll.com by the deadline to make changes to your benefit choices.

You enroll for benefits online at www.ezenroll.com.

•    When you get to the sign-on screen, you will enter your social security number (without dashes). Then enter the last 4 digits of your social security number as your password. Click the Submit button. Read the authorization statement, and then click Continue to go to the "Welcome" page. Read and follow the on screen instructions. 

•    Important: Any eligible dependent (spouse and/or children) must be entered under the "Family Information" link. It is a requirement by HSHS that within 60 days of your hire date a copy of your marriage certificate and/or a copy of your child(ren)s' birth certificate be provided to People Services. Failure to comply could result in cancellation of your dependent's benefit coverage. 

•    HSHS has a non-tobacco use wellness incentive. You must certify your and your spouse's (if applicable) tobacco use status. If you take NO action, you DEFAULT to tobacco user status and will pay higher per pay period costs for health insurance in 2017. 

•    There are certain options under the supplemental group term life insurance for yourself and/or your spouse that requires completion of Evidence of Insurability (EOI). These options are indicated (*) on the enrollment site under the Benefit Election link. If you choose an option that requires the completion of an EOI, Securian will mail this form to your home. The additional coverage elected will become effective only if you complete and return the EOI form to Securian who must approve the coverage before it can take effect. 

•    Print a copy of your online 2017 benefit elections. You will use it to compare your coverage elections to the confirmation statement located within your PeopleSoft Self-Service account. 

•    If you make a mistake or wish to change your benefit selections during the timeframe you were given above, simply sign back in and make the changes. Any changes you make before 11:59 p.m. on the last date of your enrollment period will be your elections for the 2017 plan year. 
 

If You Do Not Enroll

If you are a new hire and you do not enroll by your enrollment deadline, you will have the following coverage by default:

  • Basic life and AD&D insurance.
  • Short-term disability coverage.
  • Long-term disability coverage.
  • Basic medical plan for yourself only with after-tax deductions for your portion of the cost of this coverage. You will be defaulted to tobacco use status.

This coverage will remain in effect for the 2017 calendar year. You cannot choose new options during the year unless you have a qualifying change in status.
