Enroll for 2017!

Open Enrollment to select your Flexplan benefits for 2017 begins on October 31, 2016 and ends at 11:59 p.m. on November 14, 2016.

You will have access to the benefits enrollment website,, at work or at home 24 hours a day during the open enrollment period. You will NOT be able to access this website prior to October 31, 2016 or after 11:59 p.m. on November 14, 2016.

Important: All benefit-eligible colleagues must complete the open enrollment process for 2017 to declare tobacco use status, even if you wish to keep the same benefits you have in the current year.

Follow these simple steps to enroll online:

  • Start your internet browser and go to Enter your full Social Security number (without dashes), then the last four digits of your Social Security number for the password. Click the Submit button.
  • Read the authorization and acknowledgement statement and then click continue to go to the “Welcome” page.
  • Read and follow the on-screen instructions.
  1. Declare your tobacco status.  You MUST certify you and your spouse’s (if applicable) tobacco use status on the Tobacco Use Declaration screen. Colleagues who do NOT enroll during open enrollment will be defaulted to a “tobacco user” status and will pay higher premium costs for the entire 2017 Plan year. Be sure to click on “Submit” once you have declared your and your spouse’s (if applicable) status.
  2. Review your Personal, Family (dependents), and Beneficiary information and make any needed changes. Please include each dependent’s Social Security Number.
  3. Review your Benefit Elections and make any needed changes.
  4. If you are electing a HCRA or DCRA account, please verify the accuracy of your Direct Deposit information and make any needed changes.
  5. If you request increases in supplemental life insurance for you and/or your spouse, this will require completion of an evidence of insurability form and approval by Securian (formerly Minnesota Life) prior to the increase going into effect. Securian will mail the form directly to you.
  6. If you are eligible for PTO cash in 2017, and wish to cash in PTO during the 2017 calendar year, you must make a new election in order to receive PTO cash at 100% per IRS requirements.

IMPORTANT: PRINT A SUMMARY OF YOUR ONLINE 2017 BENEFIT ELECTIONS BEFORE YOU LOG OFF. If you make a mistake or wish to change your benefit elections, simply log back in and make your changes, then print a final summary. The last changes you make before 11:59 p.m. on November 14, 2016 will be your benefit elections for the 2017 plan year. Keep a copy of your summary to compare to the confirmation statement that you receive in December via the PeopleSoft document repository and report any discrepancies immediately to the HSHS Colleague Service Center. No corrections will be made without your Benefit Summary.
