NEW HIRE BENEFIT CHECKLIST

 Explore your benefits

               Medical, Prescription Drug, Dental, Vision, FSA Accounts, Life Insurance, AD&D, Retirement (403(b) or 401(k)).
               Benefits become effective following two full bi-weekly pay periods of Active Employment.

 Enroll in Your Benefits (Required - Time Sensitive) 

               Benefit eligible colleagues MUST elect or decline benefit coverage within 12 days of your hire date.  If you do NOT elect or decline
               coverage during the enrollment period, you will be automatically enrolled in the coverage below and the applicable premiums will be
               deducted from your pay. 
                    - Basic Life & AD&D insurance
                    - Short-term disability coverage
                    - Long-term disability coverage
                    - Basic Medical Plan - Colleague only (after-tax)
                Sign up available following your first day of employment.

               
               

 Beneficiary Designation

               In addition to your Supplemental Life Insurance options, HSHS automatically provides Basic Term Life & AD&D coverage at no
               cost to you.

 Submit Supporting Documentation

               If you are enrolling dependents under any of the HSHS insurance plans for the first time, you are required to provide proof of eligibility.
               Supporting documentation should be sent to the HSHS Colleague Service Center via fax to (217) 492-5896 or via email to
               FYI@hshs.org.
               IMPORTANT NOTICE: Wisconsin law strictly prohibits copying (including electronic copies) vital records.

               Illinois Colleagues:        Supporting documentation (Birth Certificate and/or Marriage Certificate) should be sent to the HSHS Colleague
                                                        Service Center via fax to (217) 492-5896 or via email to FYI@hshs.org 

               Wisconsin Colleagues:  Supporting documentation (Birth Certificate and/or Marriage Certificate) should be reviewed and annotated by
                                                        a local Human Resources Representative. It is the responsibility of the colleague to obtain the local verification
                                                        of eligible dependents and submit the completed verification form(s) below to the HSHS Colleague Service 
                                                        Center via fax to (217) 492-5896 or via email to FYI@hshs.org

 Discover your Retirement Benefits and Designate Beneficiary

               To encourage early investing and simplify the 403(b) Plan enrollment process, all eligible HSHS colleagues hired or rehired with HSHS
               on or after July 1, 2014 are enrolled automatically at a 4% of pay contribution level after 60 days of employment.
               Unless you choose otherwise, your contribution level will continue to increase by 1% each year until you're eventually contributing 8% of
               your pay.  These automatic contributions will be invested in the Vanguard Target Retirement Funds based on your date of birth, unless
               you choose different investment options.

               Sign up available following the first week of employment.  You can increase, decrease, or stop your contributions at any time.
