               For 2021, insurance benefits become effective following two full bi-weekly pay periods of active employment. 
               For 2022, insurance benefits will become effective the 1st of the month on or after your hire date.

 Enroll in Your Benefits (Time Sensitive) 

               The insurance enrollment period for benefit eligible colleagues will open 1 week after your hire date and will close 12 days later.  
               Be watching your email for notifications about your enrollment period and login instructions.  If you do not enroll by the deadline
               indicated in the email, you will be considered to have waived the offers of coverage.  You will not have insurance under the HSHS plans
               and will not be permitted to join the benefit plans until annual enrollment or you experience a qualifying status change. For more                               information on how to enroll, click HERE

               In addition to your Supplemental Life Insurance options, HSHS automatically provides Basic Term Life & AD&D coverage at no
               cost to you.

               If you are enrolling dependents under any of the HSHS insurance plans for the first time, you are required to provide proof of eligibility.

                Enrolling in the HSHS 403(b) plan with Fidelity Investments is a great opportunity for you to invest in your future.That's why all eligible
                HSHS colleagues hired or rehired with HSHS on or after July 1, 2014 are automatically enrolled at a 4% contribution rate after 60 days                    of employment. Unless you choose otherwise, your contribution amount will continue to increase by 1% each year until you reach 8%.
