Wisconsin Annual Enrollment Benefit Presentation

The Flexplan annual enrollment period for 2021 allows you to consider your needs and review your options to make sure you have the coverage that works best for you. Coverage will remain in effect for the 2021 calendar year unless you have a qualifying change of status.

Please Note:
During this fall’s Annual Enrollment you must enroll for 2021 benefits. If you don’t actively enroll, all of your previous coverage will NOT roll over and your current coverage will be waived for 2021.

Follow these simple steps to enroll online

  • Start your internet browser and go to www.ezEnroll.com. Enter your full social security number (without dashes). Your password is the last four digits of your social security number. Click the "Submit" button.
  • Read the authorization and acknowledgement statement and then click continue to go to the “Welcome” page.
On-screen instructions
  • Review your personal, family (dependents), and beneficiary information and make any needed changes. Please include each dependent’s social security number and date of birth.
  • Review your benefit elections and make any needed changes.
  • If you are electing a flexible spending account, please verify the accuracy of your direct deposit information and make any needed changes.
  • If you request an increase in supplemental life insurance for you and/or your spouse, this could require completion of an evidence of insurability (EOI) form and approval by Securian prior to the increase going into effect. Securian will mail the EOI form directly to you.
  • If you are eligible for PTO cash in 2021, and wish to cash in PTO during the 2021 calendar year, you must make a new election to receive PTO cash at 100% per IRS requirements.

BE SURE AND PRINT A SUMMARY OF YOUR ONLINE 2021 BENEFIT ELECTIONS BEFORE YOU LOG OFF. If you make a mistake, or wish to change your benefit elections, simply log back in and make your changes, then print a final summary. The last changes you make before 11:59 p.m. on November 14, 2020 will be your benefit elections for the 2021 plan year.

Keep a copy of your summary to compare to the confirmation statement you will receive in December via the PeopleSoft document repository and report any discrepancies immediately to the HSHS HR Service Center. No corrections will be made without your benefit summary.
