Healthy Partners

As part of your HSHS medical insurance benefits, colleagues and their loved ones receive complimentary access to a dedicated registered nurse.  Healthy Partners nurses work in collaboration with you, your support system, and your physicians to coordinate your health care needs.  Services include:

Chronic Disease Management
Assists those managing chronic illnesses (heart failure, diabetes, COPD & asthma, high blood pressure, heart disease and high cholesterol) by creating custom care plans designed specifically for your health needs, teach skills to manage health, assist with scheduling follow-up appointments, provide education on medications, and supports, guides and empower you to make educated health care decisions.

Hospital Discharge 
Provide transitional care services after a hospital discharge to assist in maintaining your health, prevent complications, and hospital readmissions.  Healthy Partners will review discharge instructions, medication, and answer any questions you may have regarding your health and recovery.  

Emergency Department Visits
Guide you through your healthcare options and the best place to seek care, saving you from expensive and lengthy visits at the Emergency Department.

Welcomed a Baby
Partner with both pediatrician and mother to ensure baby receives critically important and timely well-child checkups and vaccinations.  Your baby’s health is our focus as we partner with you for the first 18 months, guiding you through your one, two, four, six, nine, 12, 15 and 18-month checkups.

Pre-Visit Health Screenings
Preventative health screenings are effective at preventing chronic illnesses from developing.  By completing important pre-visit health screenings prior to your appointment, your physician will have reviewed your screenings ahead of time, allowing you and your physician more time to discuss what matters most to you and your health care.
